Receive assistance with groceries through Carolina Complete Health’s My Healthy Balance Value-Added Services.
Members identified with food insecurity through Carolina Complete Health’s Care Needs Screening and Social Determinants Of Health (SDOH) mini assessment will receive $120 per year (12 months from the date enrolled) for “Good for You” brand foods at Walmart.
Use your My Healthy Balance card to help pay for:
• Fruit and vegetables
• 100% whole-grain products
• Unflavored, low-fat, or non-fat dairy products
• Protein Foods (eggs, seafood, poultry and meat products)
• Fats, oils, nuts, seeds, and spreads
*This card may not be used to buy alcohol, tobacco, or firearms products.

Your card will be mailed to you once you have completed the Care needs screening and the SDOH mini assessment and are identified as having food insecurity. The card will be arrive in 2-3 weeks after you become eligible.
To request a new My Health Pays® rewards card, please contact Member Services at 1-833-552-3876 (TTY 711), Monday-Saturday 7 AM - 6 PM EST.
The card will arrive with funds loaded for the first month. Eligible members will receive $20 per month loaded on a card over 6-months.
Funds expire 90 days after termination of insurance coverage or 365 days after date reward was earned, whichever comes first.
