Meet Healthmap Solutions

Carolina Complete Health is working with Healthmap Solutions (Healthmap) to help members with kidney disease. Healthmap’s Kidney Health Management (KHM) program works with you and your doctors to help reach your health goals. 

If you qualify for the program, you can join at no cost. As part of the program, you will get help from the Care Navigation team. The team includes nurses, wellness coaches, dietitians, and social workers.
