About the LTSS Member Advisory Committee

To be eligible for the LMAC:

1.  You must be an active Carolina Complete Health member over the age of 18 or the parent, guardian, or caregiver of a member.
   
2. Member must be enrolled in Care Management Services and receive services in your home provided by a medical provider.
3. Member must have a care manager who comes to your home for visits.

The MAC will meet 4 times a year typically in March, June, September, and December. Specific dates and times will be provided prior to each meeting.

Committee members can participate in meetings in-person or virtually via computer or phone. We will provide locations and call-in information in advance of the meetings. 

• If you would like to participate in the LMAC meeting in-person and need transportation, we can provide through our vendor Modivcare (up to 30 miles each way). 
• If you would like to participate in the LMAC meeting virtually, we ask that you use the camera on your computer or phone if available so you can be fully engaged with other committee members.  If you do not have the equipment or need assistance with technology, we can help.  
• Active member participation in the LMAC is important.  We ask that each LMAC member provide feedback during the meeting.

Yes. Our transportation vendor, Modivcare, can provide transportation up to 30 miles each way. To arrange transportation ahead of a committee meeting, please call Member Services at 1-833-552-3876 (TTY 711).
Please note, transportation must be scheduled two weeks in advance.

To participate, you must have an enrollment form on file. Complete the form under "How do I sign up?" immediately to confirm your status as a registered member of the LTSS Member Advisory Committee.

• Members who attend and actively participate in the quarterly Committee meetings will receive a $75 stipend in the form of a physical gift card for each meeting, up to a total of $300.  To ensure that all members have the opportunity to participate, we limit attendance to four meetings per member or member representative. Members who continue to attend a LTSS Member Advisory Committee meeting will not receive a stipend for their participation.
  
  
• You will receive your $75 gift card mailed via USPS within 14-21 business days after the quarterly meetings, following verification of your attendance. 
  
  
• Only one gift card will be issued per household. If multiple members from the same household attend a meeting, only one gift card will be provided.
  
  
• To receive your gift card, include your current mailing address, including any apartment or suite number. If you have recently moved, it is your responsibility to register a forwarding address with your local post office.
  
  
• All attendees will receive a confirmation email detailing when to expect their gift card. Do not allow anyone else to collect your gift card on your behalf.
