The annual Care Needs Screening asks questions about your health and habits. This helps us understand how to support you. Complete your Care Needs Screening within 90 days of becoming a member, then once a year after.  Complete the screening and earn $25* on your My Health Pays® rewards card. 

*Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. Your My Health Pays Visa® Prepaid Card will be mailed to you approximately 2 – 3 weeks after you complete a healthy activities eligible for reward with Carolina Complete Health.
