Home Delivered Meals



Home-delivered meal services post-hospitalization can help support recovery and reduce readmission rates. These services provide immediate access to nutritious, diet-appropriate food, enabling recovery while reducing the physical, emotional, and financial strain on patients and their families. Members who are 18 years or older who have recently been discharged from an inpatient hospital, rehabilitation, or skilled nursing facility can receive ten medically tailored home-delivered meals up to two times per health plan year. 


Eligibility

Carolina Complete Health members must be 18 years or older to receive a maximum of two post-hospitalization meal services per health plan year after each qualifying in-patient discharge.  

Note: Please allow at least 3-4 days for review and processing of your request. Home Delivered Meals cannot be sent to P.O. boxes.

Do you have a current Care Needs Screening (CNS) on file with Carolina Complete Health?

You are unable to submit a Home Delivered Meals request because you do not have a current Care Needs Screening (CNS) on file.
