$250 Housing and Utility Assistance



Housing and Utility Assistance is available for eligible members who are experiencing housing instability due to an unexpected hardship (e.g., hospitalization, extended illness, loss of employment). Members can receive up to $250 per household, per plan year, for rental deposit or assistance; utility deposit or assistance; or mortgage assistance.


Eligibility

Eligible members must be 18 years and older and have completed a Carolina Complete Health Care Needs Screening and Housing Resource Assessment. One $250 Housing and Utility Assistance allowance per household, per year. If eligible the $250 annual household benefit will be added to a Mastercard® rewards card. for eligible members needing rental or utility assistance, rental deposit or mortgage assistance. Proof of assistance needed at the time of form submission. The rental, utility, rental deposit or mortgage assistance invoice or bill must be billed to the Carolina Complete Health member’s name.

Do you have a current Care Needs Screening (CNS) on file with Carolina Complete Health?

You are unable to submit a Housing and Utility Assistance request because you do not have a current Care Needs Screening (CNS) on file. 

If you need assistance in filling out a Care Needs Screening form, please contact your Care Manager or call Member Services at 1-833-552-3876 (TTY 711).
