Eligibility

Carolina Complete Health members must be 18 years or older and have completed two My Health Pays healthy activities prior to applying for this Value-added Service. Inactivity for more than two consecutive months will result in termination of service for the duration of health plan year. Members who cannot go to the gym due to illness, hospitalization, pregnancy or another health condition should contact Member Services at 1-833-552-3876 (TTY: 711) to avoid Value-added Service cancellation.
