On April 22, 2024, UnitedHealth Group issued a press release, providing an update on the Change Healthcare cybersecurity incident that occurred on Feb. 21, 2024. Given the size of the data impacted, the investigation to determine whose data is impacted is expected to take several months. UnitedHealth Group believes this situation will impact “a substantial proportion of people in America” and is offering immediate credit monitoring and identity protection services, as well as a dedicated contact center to address questions. Youth Programs


After school sports and youth club programs can help children learn new skills, create new friendships, and improve overall mental and physical health and wellbeing. That's why Carolina Complete Health members get one voucher per year for after school sports and activities or youth club memberships valued at up to $75, ages 6 to 18. To get this Value-Added Service, please complete the form below. Legal guardian must complete form on behalf of member.

A $75 voucher for Youth Programs for members age 6-18. Limit of one voucher per year per member.

Carolina Complete Health is not responsible for making sure there are openings for afterschool activities/sports programs. Carolina Complete Health is not responsible for paying extra costs for programs beyond the $75 voucher. Vouchers do not have a cash value and cannot be redeemed for cash. Vouchers cannot be transferred to another person and will expire one year from the date they are issued.

Eligibility Statement: Eligibility requirements apply. You must be a current Carolina Complete Health member to receive this Value-Added Service. One voucher per child every 12 months. Must complete separate form for each child.
